The problem grows as AI systems are adopted in more sectors. Health insurers use algorithms to determine eligibility for coverage. Law enforcement agencies deploy predictive policing tools. Autonomous drones and military technology rely on machine perception to distinguish civilians from combatants. In all these contexts, hallucinations pose not just an inconvenience, but a potential threat to life, liberty, and justice.

Distinguishing between creative generation and factual error is crucial. When an AI is used for entertainment or brainstorming, unexpected outputs can be part of the charm. But when a tool is expected to deliver truth and it fabricates details with authority, that’s when trust breaks down.

Ultimately, no matter how advanced these tools become, human oversight remains essential. Users must be cautious and skeptical, especially when an AI output influences serious decisions.
